Certificate Requests

A certificate of insurance is a document from an insurance agent/broker that outlines the types and limits of insurance carried by an entity. All requests for certificates must be forwarded to the Office of Risk Management.

  • A copy of the Request for Certificate of Insurance form should be submitted (available through the Office of Risk Management);
  • A copy of the contract/agreement that includes the insurance requirements should be submitted with the request; and
  • Requests should be made at least 3 to 5 business days in advance.
