Sign the "Blue Form"

Come in to the SCDC to sign the Student Agreement Form (or "blue form"). This form is extremely important in that it notifies you of your final co-op placement and salary.

Independent Job Search & the "Blue Form"

The "blue form" is equally important for co-op student conducting their own independent search (outside of SCDConline). When you receive a job offer, bring a copy of both your offer letter and the job description to the SCDC. You will submit these documents along with a completed blue form. These documents will formalize and allow you to receive credit for your co-op.
