Important Notes

On these forms, “Program Director” refers to DCT. (i.e., the signature of the DCT is required.)
When forms are complete, students should make a copy for their own records and then submit to the Ph.D. program’s administrator, currently Damaris Oquendo. The administrator will make a copy for the student’s file, and then submit the original to graduate studies. (The student should not submit to graduate studies directly.)
Ph.D. students who entered the program with a Masters degree should complete all doctoral forms except those specifically related to the Masters thesis, and should indicate that they have a Masters degree on the D-1 form.
