Course Add/Drop/Withdrawal
Use this form to change your registration during the current term. Classes can be added or dropped for only the first two weeks of the quarter, and graduate students may withdraw from a class through the end of the sixth week of the quarter, with proper signatures from departmental advisors. -
Transfer Credit Application
Any student wishing to transfer credit from another institution must use this form to obtain permission. Once the academic department has decided to award transfer credit, submit the complete form to Graduate Studies for processing.

Application for Degree
As part of the steps to graduate, all students must complete the Student Resource Center’s online Application for Degree. Submitting the application notifies the University of your intent to graduate and initiates a check to make sure you have met all degree requirements. Thesis Approval
This form must be completed by all students with a thesis requirement and will be included in the bound thesis filed with the library. -
Completion Form
Graduating students must complete this form by the first day of final exams during the term in which you complete your studies.
Survey of Earned Doctorates
The Survey of Earned Doctorates is an annual survey of all individuals who complete a research doctorate at a US university. Graduating PhD students should submit this completed survey to the Office of Graduate Studies.
Drexel PhD Exit Survey
This form is to be completed by all PhD students upon leaving Drexel University.
Program Forms

Accelerated Program Withdrawal Form
Use this form to withdraw from an accelerated program. -
BS/MS Program Application
Use this form to apply to all non-business Bachelor’s-Master’s Degree programs. Submit the form to the appropriate departmental personnel for approval and finally to Graduate Studies for processing. -
BS/MBA Program Application
Use this form to apply to all Bachelor’s-Master’s Degree programs within the LeBow College of Business. Submit the form to the appropriate departmental personnel for approval and finally to Graduate Studies for processing. -
Major/Program Transfer Application
This form is to be used by any graduate student applying for permission to transfer from one major or program to another within the University. After departmental approval signatures are obtained, submit it to the Office of Graduate Studies for processing. -
Dual Master’s Degree Application
This form is to be used when applying to complete and receive two Master’s degrees. Submit the form to the appropriate departmental personnel for evaluation and then to Graduate Studies for processing. -
Application for Readmission
A student who has previously been accepted or enrolled at Drexel and wishes to re-enroll in a graduate program should use this form to request readmission. -
Change of Matriculation Status
Non-matriculated students must use this form to declare their intent to matriculate into a degree program or to continue studying in their non-matriculated status before registering for more than 12 credits.
Leave of Absence
Graduate students who wish to formally request a leave of absence for less than one academic year (four consecutive quarters).

Withdrawal Form
Graduate students who wish to formally withdraw from the institution, effectively severing their relationship with Drexel University. - Change of Curriculum and Status
Graduate students who wish to change their academic program, concentration, and/or status from part time to full time or full-time to part-time.

Full-Time Exemption Request
This form is to be used to request an exemption from full-time enrollment status (9 credit hours per quarter) for pre-doctoral students. This form will not be accepted in the Office of Graduate Studies until the 2nd Week after the start of each quarter.
Doctoral Student Forms

Form D-1: Plan of Study
A plan of study, incorporating all courses and research credits, will be formulated; after approvals are received from the student's supervising professor, graduate adviser, and department head, the plan will be sent to the Office of Graduate Studies for final approval. -
Form D-2: Appointment of a Supervising Professor
A supervising professor should be appointed using the Form D-2 by the end of a doctoral student's second year of study. -
Form D-3: Doctoral Candidacy Committee Appointment & Exam Schedule
The Form D-3 must be filed with the Office of Graduate Studies at least four (4) weeks prior to the scheduled date of the examination. This form specifies the required composition of the Doctoral Candidacy Committee and documents the committee members. Please follow specific committee composition requirements on the form. -
Forms D-4 and D-4a: Reports on Candidacy Examination
Students should bring Form D-4 and copies of Form D-4a for each committee member to their examination. -
Form D-5: Appointment of the Thesis Advisory Committee
The Form D-5 should be filed prior to the proposal defense. This form specifies the required composition of the Thesis Advisory Committee and documents the committee members. Please follow specific committee composition requirements on the form. -
Form D-6: Appointment of the Final Oral Defense Committee
This form is used to schedule the final defense of the dissertation. It is to be filed with the Office of Graduate Studies at least four (4) weeks prior to the final defense date. -
Form D-7: Report of the Final Oral Defense Committee
This form must be filed with the Office of Graduate Studies within 48 hours after the completion of the final oral examination. This form documents the results of the final dissertation oral defense. Students should bring Form D-7 to their examination.

Travel Subsidy Application
All graduate students who will be presenting at a conference are eligible for a small subsidy award to help defray the cost of travel. Submit this form to Graduate Studies with all supporting documentation at least one month in advance of the conference for consideration. -
Provost Fellowship Nomination
This form is to be used by Drexel faculty or staff to nominate an outstanding new doctoral student for an additional fellowship award.
