  • Complaint Form for Discrimination and/or Harassment [PDF] - Submit a complaint of discrimination/harassment to the Office of Equality and Diversity.
  • EEO Compliance Form [PDF] – Hiring bodies must submit this completed form to Human Resources for all posted positions before an offer can be made.
  • Request for Waiver of Job Posting Requirement [PDF] – Use this form to request that search procedures be waived for a specific job applicant.
  • Voluntary Self ID Form - The University is required to report to the federal government summary data about the gender, ethnicity, race, and veteran status of its employees and its efforts to achieve equal opportunity through affirmative action for minorities, women, persons with disabilities, and veterans.

    To self-identify your gender, ethnicity, race, or disability or veteran status, Please do the following:

    2. Select Drexel tab
    3. Under Personal Information, select Race and Ethnicity Information.
