- Transcripts from all colleges and universities attended
- Essay: Describe your career goals and the values and experiences that led you to make those choices. Explain your topic of special interest and describe how this fits with the mission of the program in Health Policy and Social Justice.
- Three letters of recommendation (one must be from a public health practitioner)
- GRE
- Documented evidence of applied research
- Résumé
For international students or applicants who earned a degree outside of the US
- Minimum TOEFL – 580/237 – iBT scores will be reviewed based on section scores and total scores
- I-20 Application and supporting financial documents
- International transcript evaluation
