- Transcripts from all colleges and universities attended
- Essay: Describe your career and educational goals, professional experience, and area of interest for your dissertation.
- Three letters of recommendation (one must be from a public health practitioner)
- GRE
- Documented evidence of applied research
- Résumé
For international students or applicants who earned a degree outside of the US
- Minimum TOEFL – 580/237 – iBT scores will be reviewed based on section scores and total scores
- I-20 Application and supporting financial documents
- International transcript evaluation
