Exam/Quiz Accommodation Form
Instructions (for Students and Faculty)
The Testing Accommodation Form has three separate areas of completion. The form is available on-line at Exam/Quiz Accommodation Form, or available in the Office of Disability Services (ODS).
- The first section needs to be completed by students with disabilities, who are eligible for accommodations at Drexel University. Students are responsible for initiating the exam accommodation process with faculty members and can submit a paper form, or e-mail it electronically, if they choose.
- The second section needs to be completed by the faculty member of the course. After completion, the faculty member needs to mail the paper form to ODS or e-mail the form to both the student and ODS. For additional information regarding mailing documents to ODS, e-mail disability@drexel.edu.
- The third section is completed by an ODS staff member. All Testing Accommodation Forms are filed at ODS, and maintained as confidential information. Exam/Quiz Administration in the
Office of Disability Services (for Students)
When students take exams in the Office of Disability Services (ODS), there are policies and procedures which must be followed. The intent of having exams/quizzes administered at ODS is to allow the use of accommodations, while replicating the classroom structure.
1. Unless noted by the faculty member on the Exam/Quiz Accommodation Form, or as an approved accommodation, no academic or personal items are allowed during the exam administration. All student materials will be held at ODS during the administration. Cell phones must be turned off and cannot be used during the administration.
2. Students suspected of cheating, will be reported to the faculty member of the course. Action will be taken in accordance with the University’s judicial policies and procedures.
3. All students are expected to arrive on time, as the starting time of the exam begins at the scheduled time on the Exam/Quiz Accommodation Form. If students arrive at a later time, the amount of time will not be extended beyond that of what was originally scheduled. If students wish to reschedule exams/quizzes, all requests need to be approved by the faculty member of the course. If a student chooses to request to reschedule an exam due to late arrival, the faculty member of the course may not allow the exam/quiz to be taken at another time. If this occurs, Drexel University’s polices and procedures will be applied, as the administration of exams/quizzes for all students.
4. In instances where students are allowed to take breaks, as a reasonable accommodation, students are not allowed to leave ODS or access personal information/backpacks. Unless otherwise noted on the accommodation verification letter (AVL), time allotments for breaks will be factored into the maximum amount of time allowed for the exam/quiz. Maximum amounts of time will be listed on the AVL, if applicable.
Exam/Quiz Administration (for Faculty)
1. The accommodation verification letter (AVL) will list the accommodations for which the student is eligible. The AVL is issued by the Office of Disability Services (ODS) on a per term basis. Students need to share an AVL for the current term, in order to be eligible for accommodations. Also, students need to share an AVL with their faculty members for each course they are enrolled in, rather than submitting an AVL for the entire term/period of study. For example, if a student has the same faculty member for more than one course, or multiple terms, a current AVL needs to be shared with the faculty member for each class, each term.
2. Exam/quiz accommodations can include a multitude of accommodations. The intent of a reasonable accommodation is to minimize the impact of the condition, while allowing students with disabilities to demonstrate their abilities. There are accommodations which are more common than others, largely because accommodations such as, extended time minimize the impact of many types of disabilities for diverse reasons.
3. A reduced distraction testing environment allows a student with a disability to take the exam/quiz in a room alone, except for exam proctoring. The intent of this type of accommodation is to minimize extraneous noises and visual cues and aids, but we cannot guarantee all noise will be removed.
4. The need for alternative formats during the exam administration can be a timely endeavor for ODS to facilitate. Although ODS will make every effort to meet the needs of the University, faculty need to submit exam materials to ODS at least 48 hours prior to the exam administration. ODS will provide all converted materials for the exam/quiz administration.
5. The need for adaptive technologies during the exam administration may require students with disabilities to take exams/quizzes in a specific location of the department, or ODS. If technology is networked, it allows students and faculty the flexibility for the exam administration to occur in multiple locations, rather than only ODS.
