This Departmental
form must be filled out by any Department Head or Dean requesting
to relocate staff or faculty, expand or reconfigure existing
space or alter the way a space is used (e.i. converting offices
into a classroom).
Failure to
use this form may result in loss of space or department expense
to cover new work.
However, you should not use this form for general maintenance
and repairs, such as repainting, replacing carpeting, fixing a leak, or adding electrical outlets. If you
need general maintenance services such as these, please complete a Work Order Request and e-mail
it to Facilities Management at fixit@drexel.edu |
