Technology Award: Preparing Your 2008 Nomination

In the body of your email, include your name, address, telephone
number (day-time), fax number, email address and indicate whether
you are a nursing educator or practicing nurse.

Narrative
Describing:
- The technology and how it is used,
- The target group impacted,
- The impact and benefits of the technology,
- The technology must be in use six months or longer
No
more than one 8.5" x 11" page with one-inch margins all around.
Use 10 point, Times New Roman font, single-spaced. Prepare
this narrative in Microsoft Word.
Please
do not put your name or the name of your institution on the
narrative.

supporting letter from the Dean or Chair of Nursing
if you are an educator or the Chief Nursing Officer of your
institution/agency

Two
additional letters of support stating why the technology
is important and innovative
