Forms & Resources

The following forms must be used to report information and register events:

• New Member Registration Form - This form is due no later than 48 hours after the members accepts the invitation to join. Must be submitted to 215 Creese Student Center with Anti-Hazing form, stapled and in alphabetical order. Attach a full list of new members to this packet including name, ID number, and email address.

• Hazing Acknowledgement Form for New Members - This form is due no later than 48 hours after the members accepts the invitation to join. Must be signed and submitted to 215 Creese Student Center with New Member Registration form, stapled and in alphabetical order.

• New Member Education/Intake Plan Cover Sheet - This form is due Friday, September 2, 2011 along with the complete organization New Member Education/Intake Plan.

• Panhellenic Continuous Open Bidding Membership Recruitment Acceptance Binding Agreement (MRABA) - Required for all new members accepting membership into Panhellenic sororities outside of formal recruitment to be turned in with new member forms.
  
  
• Officer Update Form- This form details all chapter officers and advisors and is due September 20, 2011. Please send electronically to greeklife@drexel.edu
  
  
• Semi-Annual Goals- This form is due Tuesday, September 20, 2011 by 5pm and should detail the chapter's direction and focus through the Winter 2012 term. Please send electronically to greeklife@drexel.edu.

Programming Verification Forms - All programming hours are due to greeklife@drexel.edu by the 2nd Friday of the following term.

• Request for Contract - Use this form to capture ALL information that will be needed in the contracting process and return to 215 Creese Student Center or the Student Organization Resource Center in the basement of Creese Student Center. This is not a contract, but this information will be used to create a contract.
