Requesting Service Providers

A request form needs to be completed for the receipt of auxiliary aids, accommodations, and services by students and employees who are deaf or hard of hearing. The request forms will allow for a smoother receipt of services in a timely fashion. Students and employees who have disabilities need to follow the policies and procedures for requesting accommodations and registering for services with ODS.  For additional information regarding requesting accommodations due to deafness or hard of hearing, go to: Requesting Auxiliary Aids, Accommodations, & Services.
