Equipment / Material Loan

There are two forms necessary for checking out alternative formats through the Office of Disability Services (ODS). The forms are available on-line or at ODS, upon request. 

• Equipment/Material Agreement Form
• Equipment/Material Request Form

Both forms can be printed and submitted to ODS by one of the following:

1. Mailing the completed form to ODS at 3141 Chestnut Street, 81-210, Philadelphia, PA  19104.
2. Faxing the completed form to ODS at 215.895.1402.
